2. Fill in the personal

of the application
form. All fields
must be
entered.
If not applicable
indicate N/A.
3. Select the certified
documents you will
provide in
your application
interview.
4. Indicate
the operational
need that best
suits your situation.
You will need to
provide this in
the form of a letter
at the time of
your application
interview.
5. Please choose
the method of payment
for your MSIC which
will be paid at
the time of your
application interview.
A single card costs $165 inlcuding GST.

6. Please indicate
if you require
us to take a photo.
7. Once you have
completed the information
click on the “submit
application” button
on the application
form.
